Job Requirements of MANAGER OF PATIENT FLOW, GBGH@HOME, & FLOAT TEAM:
QUALIFICATIONS & EXPERIENCE
The ideal candidate will possess the following key competencies, record of experience and academic preparation:
- Bachelor’s degree or higher in a health care-related field such as Nursing, Social Work, or Health Administration.
- Proficient with CNO standards of practice.
- Minimum five years of clinical experience in Acute Care or Rehabilitation/Complex Continuing Care Hospital settings.
- At least one year in a leadership role, with a preference for three years of recent managerial or leadership experience.
- Experience with Lean Health Care improvement methods and a commitment to continuous professional development through education and professional associations.
- Proven ability to resolve human resource issues through facilitation, collaboration, and negotiation.
- Comprehensive knowledge of legislation, labour relations, collective agreements, medical-legal issues, and Ontario Health's role and priorities.
- Experience engaging community partners and stakeholders.
- Proficiency with Microsoft Office Suite and strong email management skills.
- Demonstrated change management skills and ability to manage competing priorities effectively.
- Strong communication and interpersonal skills, with the ability to collaborate effectively within interdisciplinary teams and across all levels of the organization.
- Successful leadership experience in fostering teamwork, building relationships, and achieving stakeholder consensus.
- Dedication to client service and a commitment to diversity and inclusivity.
- Professional demeanor, with respect for protocol and confidentiality.
- Embraces person-centred care and GBGH’s Values and Purpose Statement.

THE OPPORTUNITY
As the Manager of Patient Flow, GBGH@Home, and the Float Team, you will play a key role in ensuring efficient patient flow from GBGH care units to discharge destinations, while supporting primary nursing care functions. Reporting directly to the Director of Clinical Services, you will oversee patient care and day-to-day operations, including administrative, human resources, and financial activities. You will also be responsible for aligning organizational initiatives with GBGH's strategic direction, relevant legislation, corporate policies, and collective agreements, while addressing risk and safety concerns and fostering a positive approach to change.
In this leadership role, you will provide oversight for the GBGH@Home Program, enabling patients and families to access a co-designed care plan that supports thriving in the community. Additionally, you will manage relationships with external hospital partners to enhance patient flow and repatriation efforts, while ensuring effective operations of the Float Team to meet organizational needs. Your role includes monitoring budgets, tracking patient care statistics, supporting quality improvement initiatives, and advancing patient and staff safety programs.
As a role model, you will coach and mentor staff to foster teamwork and professional development, oversee hiring and performance management, and strengthen interdisciplinary collaboration. Through these efforts, you will ensure that policies, procedures, and initiatives align with GBGH’s strategic objectives and promote high-quality, person-centred care.
